There are some known issues with the conversion of the 15.4.10k upgrade. The issues reside in the actual conversion program and not in your forms file. It appears that the upgrade tries to convert your forms file from Access 2000 to Access XP. This causes the forms file to become corrupt. This does not happen on all machines, so it is important that you do the conversion on all of your PCs, then test out the forms printing. Front Office forms are affected as well as ALPs forms.

How to fix
 
First, do not reload your forms file! Run the conversion program on your existing custom  forms. After the conversion, test our your forms.

 

Run the conversion on all of your PCs. Then test out the forms to make sure they print OK. You may find that they print OK on some machines, but not all. If you have a machine that is working OK, then copy the ALPSFORM.MDB file from that machine to the machine that is not working properly. Same goes for FOFORM.MDB (your front office forms file). The files are located in C:\FSP\MDB\

If you don't have any success printing forms on any of your machines, then you'll need to send a copy of your forms file to either HFS Enterprise Solutions (f.k.a. PSI) OR if you are an in-house system, then to ULTRADATA directly. If you need help doing this, please let us know and we will send a copy of the file for you. They will then correct the database and send it back to you.

PSI ... "has found that sometimes if the CU receives the run time error 3078, they can reinstall their forms on the PC and reload the FSP upgrade CD and it resolves the problem.  This, of course, is not a consistent fix!!"
 

It is important to remember that this is a conversion related problem and not a problem that we, Forms Design, Inc.,  can fix.  You will need to work directly with either PSI or ULTRADATA on resolving this issue. Obviously, you would be prudent to do the conversion and test your forms as soon as possible so that they can help you resolve any issues that may arise.
